"It's been a while since I lost my lower molars. It's difficult to eat, so I want to be able to chew properly. My teeth are also starting to get crooked, so I want to straighten them out nicely," she said in her consultation.

Counseling and diagnosis results

Upon examination, we found that the teeth were all crooked due to a mismatch in the size of the alveolar base, the bone on which the teeth are aligned. This caused crowding, and we also found that the upper and lower front teeth overlapped too deeply, resulting in an overbite where the lower teeth were not visible.

下の奥歯5本(右第2小臼歯/5番、左右第1大臼歯/6番、左右第2大臼歯/7番)が欠損したまま長く放置されていたことが原因で、噛み合う歯がなかった左上の奥歯(第1大臼歯)は本来の位置より下に伸びる「挺出(ていしゅつ)」が起き、左下の歯2本(第1小臼歯/4番、第2小臼歯)も内側に倒れたことで、下前歯の叢生が悪化したと考えられます。

If left untreated, this condition can lead to poor digestion and absorption, as well as the inability to chew food properly, and can put strain on the jaw joint, leading to temporomandibular joint disorder.

Furthermore, the missing tooth on the upper left (first premolar) had been fitted with a bridge, a cap that connected an artificial tooth to the two adjacent teeth (canine/number 1, second premolar) as a base. Not only was the silver filling visible when the patient opened their mouth, but the overall colour of the teeth was yellowish, making it look unsightly.

The patient wanted to stabilize her bite and improve her appearance by adjusting the color and alignment of her teeth.

Suggestions and treatments provided

The ideal alignment of teeth is called "anterior guidance," in which when the lower jaw is moved forward and to the side, only the backs of the upper front teeth and the tips of the lower front teeth come into contact, with a gap between the molars. In the patient's case, the following three treatments were required to achieve proper anterior guidance and enable him to chew food properly.

1. Replace missing molars
2.Improvement of crowding
3. "Bite lift" to return a deep bite to the appropriate height
4. "Intrusion": Depressing the protruding upper left molar (first molar) into the gums

Since orthodontic treatment is difficult when all of the back teeth are missing, we first performed implant treatment to implant artificial tooth roots to achieve ①, and then proposed orthodontic treatment for the entire mouth to treat ② to ④, which the patient agreed to.

First, in order to determine the correct position where the implant should be inserted and the ideal goal, a simulation is performed using a "setup model" in which the teeth of a model made based on the patient's teeth are disassembled and rearranged in anticipation of the teeth alignment after treatment.

Implants were placed precisely in the positions determined by simulation for the four lower molars (right second premolar, left first molar, and left and right second molars).
Next, temporary teeth are fitted to the two teeth that were the base teeth for the upper left bridge (canine and second premolar) and each of the four teeth with implants, and then orthodontic devices are attached to the front of all the teeth, both upper and lower.

Orthodontic treatment improved crowding, raised the bite, and reduced the protruding upper left molar (first molar), providing appropriate anterior guidance, so the orthodontic device was removed.

半年間経過を観察したあと、左下のインプラント2本(第1大臼歯、第2大臼歯)にはそれぞれ単独の被せ物、右下の奥歯3本(第2小臼歯、第1大臼歯、第2大臼歯)にはインプラントを土台にしたブリッジ、左上の歯3本(犬歯、第1小臼歯、第2小臼歯)には歯の大きさを調整して犬歯と第2小臼歯にそれぞれ単独の被せ物を装着しました。

We choose "ceramics" as the material for the crowns, which have the transparency of natural teeth and are both aesthetically pleasing and durable, depending on the patient's wishes.

In addition, to improve the yellowing of natural teeth, we performed "dual whitening" on the entire mouth, which combines "office whitening" performed at a dental clinic with "home whitening" performed at home. The advantage of dual whitening is that it achieves the ideal whiteness in a short period of time and is less likely to revert to its original color. The treatment was completed with one office whitening session and approximately three months of home whitening.

Postoperative progress and current condition

By having the implants placed, I can now chew food properly. The yellowing of my teeth has improved, and my teeth are now aligned, giving me a beautiful mouth again.

The patient was very satisfied, saying, "I'm glad I was able to avoid dentures in my 40s. I can now chew anything, and I'm very happy that my appearance, which had been bothering me, has improved."

General risks and side effects associated with implant treatment

  • - Because the emphasis is on functionality and aesthetics, treatment is at your own expense (not covered by insurance) and is more expensive than insurance-covered treatment.
  • A surgical procedure is required to place the implant.
  • -People with high blood pressure, heart disease, asthma, diabetes, osteoporosis, or kidney or liver dysfunction may not be able to receive treatment.
  • - Pain and swelling may occur after surgery, but in most cases this will subside within a week.
  • After surgery, you may experience temporary numbness in your gums, tongue, lips, and cheeks. You may also experience inflammation, pain, delayed tissue healing, and facial internal bleeding in your jaw, nasal cavity, and maxillary sinuses (cavities on both sides of your nasal cavity).
  • -After surgery, side effects such as drowsiness, dizziness, and nausea may occur due to taking medication.

    • - Attended specialist education courses three or more times.
    • -Has participated in the association's academic conference and branch academic conference more than eight times.
    • - Have completed the required training as stipulated in the implementing regulations.
    • - Recommendations have been received from two oral implant specialists (one of whom is the facility director).
    • - Have experience in treatment using implants as specified in the implementing regulations.
    • - Pass the case presentation exam.
    • - Have made presentations at least twice at the Society's academic conference or branch academic conference.
    • - Published at least one paper on oral implants in the journal of the Society or in a foreign journal approved by the committee.
  • Conditions for holding the qualification of "Oral Implant Specialist"
    • ・Renew your qualification every five years.
    • - After discussion by the committee, it will be updated by the board of directors.
